We have dog's name, our address, and our phone number on Ellie's tag. Although I wouldn't swear to it, I would think that would be sufficient to satisfy most requirements. Your local Humane Society and maybe even your vet should know the requirements for your area.

Hmm, I don't think it's a requirement everywhere, but it's definately a very good idea.

On Maddi's I have her name, my name, and my cell number.

It's not a requirement here. The only requirements are license and rabies tags. Though, after Zoe had escaped the first time, and I found her avoiding these 2 men they said "We were just trying to catch her to look at her tags." And I realized, if that happened, she would have been brought to the local shelter, cause she had no ID on her. So after that, I got her one with her name, my address, and phone number. She got out again after that, and these 2 kids had caught her, and called 20min after she'd escaped. They're really quite handy :D

My dogs tag doesnt have her name on it, it has my mobile number and it says "Scan me"(for a microchip).

never ever put your dog name on the the tag because if your dog does get lost and someone find him they can see the name and more then likely the will keep the dog

they can use the name to see what the dog knows and if it is well trained are more apt to keep it. i always just put "if found call #"

if that person does really like your dog and wants to keep it that person will know the dogs name and your dog will come to the person
